Prenatal & Early Family Health Education 

Overview: Localized version of Black Creek model, supporting infant health and early parenting. 

Topics: Infant vaccine schedule (2, 4, 6 months), caregiver vaccine (Tdap), Mental health & parenting support, Local health resources.

Target Audience: Expectant parents & caregivers; New parents; families in local catchment.

Key Benefits: Builds trust with care team; preventative care; connects families to resources; space for Q&A. 

Schedule / Format: 2 sessions; 45 minutes each; twice monthly, hybrid delivery (In Person & Zoom); complements Black Creek's program. 

Facilitators: Registered Nurse, Nurse Practitioners & Social Workers.
