History
On February 6, 2007 nineteen physicians signed an agreement with the Ministry of Health to form the Humber River Family Health Organization. On March 4, the group offically created and signed the Association for the Humber River Family Health Organization.
In 2010, the Ministry of Health was creating Family Health Teams to assist Family Health Organization to provide care to their patients. Dr. Kushner, a family physician and Chief of Family Medicine at Humber River Hospital, inconjuction with Barb Willits, Program Director for the Humber River Hospital, created a Business and Operations Plan.
On Feburary 11, 2011 the Humber River Family Health Team submit the proposal to the Ministry of Health. The Ministry approved the creation of the Humber River Family Health Team on April 01, 2011.
The Family Health Team is made up of different health care professionals, to provide care and added support to patients and their families.
On May 20, 2011 the Humber River Family Health Team received their Letters of Patent. The application was signed by three Humber River Family Health Organizaion (FHO) physicans, Dr. A. Kushner, Dr. R. Masi and Dr. J. Crummey.
The first Executive Director was hired on August 4, 2011.
Presently members of the Family Health Team see patients at our Dixon & Islington, Mount Dennis & Weston, Wilson & Keele, Finch & Keele and Finch & Weston locations.

Who We Serve
HRFHT is geographically located within the North WesternToronto Ontario Health Team (OHT) and provides health care services to patients from various parts of Toronto and the GTA.
Under the current funding model the Family Health Team provides care to all patients who are rostered with an associated physician. With the Patients First Act, this care delivery model may change, allowing us to serve all members of the community.
