Pre-Diabetes & Diabetes Program
The Diabetes Program offers patients education and counselling to help you manage your diabetes and prevent complications.
The Diabetic team can help you with:
- Foot assessment and care
- Nutrition education and planning
- How and when to take your medications
- The importance of monitoring your blood sugar
- Self care education and planning that is specific to you
Our Registered Dietitians are also Certified Diabetes Educators who can assist you with diabetes management. Our services include insulin start and adjustment, Ozempic/Mounjaro/Trulicity/Victoza start, glucometer teaching, Libre 2 sensor and Dexcom application.
Living Well with Diabetes class is a 2-part session offered in person. Patient and community members are invited to attend these classes. You do not need a doctor's referral to attend these free classes.
- Part A (For those Newly Diagnosis) April 02, 2025
- Location: 245 Dixon Rd (Dixon & Islington). When you arrive, please check in at POD 1.
- Time: 6-7:00pm

- Time: 6-7:00pm
