Health & Nutrition Program
At Humber River Family Health Team our Registered Dietitians offer both group and one-on-one counselling sessions to assist patients with a number of different health concerns including:
Registered Dietitians can help you meet your health goals by translating the science of nutrition into terms you can understand.
Our service is also available in-office to assist individuals living with home tube feeding.
For patients with dyslipidemia and hypertension, we offer a 2 hour workshop on Introduction to the Mediterranean Diet which covers key concepts of the Mediterranean Diet including how this dietary pattern can help manage hypertension and dyslipidemia. Portion control and Label reading is also covered in this workshop.
Our Registered Dietitians are also Certified Diabetes Educators who can assist you with diabetes management. Our services include insulin start and adjustment, Ozempic/Mounjaro/Trulicity/Victoza start, glucometer teaching, Libre 2 sensor and Dexcom application.
Patients, their caregivers, and community members are invited to attend this FREE Mediterranean Diet Class.
