Health & Nutrition Program
At Humber River Family Health Team our Registered Dietitians offer both group and one-on-one counselling sessions to assist patients with a number of different health concerns including:

Prenatal Nutrition
Registered Dietitians can help you meet your health goals by translating the science of nutrition into terms you can understand.
Our service is also available in-office to assist individuals living with home tube feeding.
Our Registered Dietitians are also Certified Diabetes Educators who can assist you with diabetes management. Our services include insulin start and adjustment, Ozempic/Mounjaro/Trulicity/Victoza start, glucometer teaching, Libre 2 sensor and Dexcom application.
For patients (or participants) with dyslipidemia and hypertension, we offer a 60 minute Heart Health workshop.
This session is for participants with high blood pressure who are interested in managing their health condition through diet.

For patients (or participants) who are interested in healthy eating for weight loss, we offer a 60 minute Mediterranean Diet Class.
Patients, their caregivers, and community members are invited to attend this FREE.
