Acute and Episodic Care
Acute and Episodic care consists of procedures ordered by your physician or nurse practitioner.
The program aims to:

Improve timely access for acute and episodic care.

Improve fragmentation and gaps in Health and Service delivery.

Promote and facilitate continuity of care; identifying and removing barriers to care.

Assist patients to navigate multiple health, and social services
- Provide same day appointments.
The program is supported by our Registered Nurse, Nurse Practitioner and Pharmacist all with same day appointment in their schedule.
Patients of the Humber River FHT can have these procedures completed by members our health care team.
They include such procedures as:

B12 injections

Wart treatment

Adult immunizations

Allergy injections

Suture/staple Removal

Ear Flushing, etc
