We have partnered with Humber River Hospital to provide a vaccination event: On Friday October 14 2022: 9am to 1pm 245 Dixon Rd, Etobicoke (Free Parking)

Offering: Moderna Bivalent COVID-19 Booster & Pfizer Vaccines 

Eligibility:

  • 1st Dose: Ages 6 month & older. Children must be 6 months at the time of vaccination.
  • 2ndDose: Ages 6 months & older. 8 weeks’ interval between dose #1 & #2.
  • 3rdDose (Booster#1): Ages 12-17 (168 days) 6-month interval between dose#2 & #3. Children must be 12 years old at time of immunization
  • 1st or 2nd Booster-Moderna Bivalent Covid-19: Ages 18+. Interval must be 3 to 6 months from the last Covid-19 vaccine or after 3 to 6 months from testing positive for Covid-19 or having started symptoms

While we hope you do not need to make a complaint about the care and service you have received, we aim to make the process easy. Complaints and its process will not affect the quality of care you receive from members of the Family Health Team.   The complaint will be confidential at all time during the process.

Please remember,

  • You will need to present your valid health card at your first visit and we will ask you to complete an enrollment form at that time if you are a new patient.

Please support us by honoring your appointment times.

To help us run our clinic smoothly, please do your best to be on time for your appointment.

 If you do not call 24 hours prior to your appointment, or do not show, it will be counted in your file as a no show.

Once you have 2 consecutive or 3 total no shows on record you will not be allowed to rebook/reschedule and your doctor will need to be notified.
