The Lactation Consultant is available 2nd, 3rd, 4th Wednesday of each month 11 am– 6:00 pm @ 2050 Weston Road for individual appointments.

A Prenatal Breastfeeding support class will be held on Wednesday Dec 08 6-8pm @ 2050 Weston Road.  This FREE service is available to all families with or without OHP / Health Coverage.

The direction of the the Humber River FHT is to work closely with the Central LHIN, Humber River Hospital, and our community partners in the North York West sub-region to enhance system and service navigation for seniors living alone and with chronic medical conditions. 

Our program and services are delivered in a team based care model including NPs, RNs, SWs, Chiropodists and Pharmacist who provide care and support to our patients.

  • You will need to present your valid health card at your first visit and we will ask you to complete an enrollment form at that time if you are a new patient.

Please support us by honoring your appointment times.

To help us run our clinic smoothly, please do your best to be on time for your appointment.

 If you do not call 24 hours prior to your appointment, or do not show, it will be counted in your file as a no show.

Once you have 2 consecutive or 3 total no shows on record you will not be allowed to rebook/reschedule and your doctor will need to be notified.
