Chronic Condition Eduation Series
Overview: Rotating sessions on self-management for chronic disease, Diabetes; COPD & Asthma, Hypertension & cardiovasccular health. Includes blood pressure control, medication adherence, heart-healthy living and respiratory care.
COPD/Asthma patients may also access services from a Pharmacist (Spirometry) and a Nurse Practitioner (vaccinations).
Target Audience: Participants with chronic onditions; adults managing multiple conditions; caregivers.
Key Benefits: Inclusive for multiple conditions; encourages open discussions; builds patient confidence; links to follow up care.
Schedule / Format: 60 minute per class; 3 rotating topics (weekly); every other month; hybrid model (In person & Zoom).
Facilitators: Dietitians, Nurse Pracitioners, Pharmacist.

- Part 2 COPD/Asthma (10:00-11:00AM) ~ Tuesday November 18, 2025 ~ 2050 Weston Road (and Zoom virtually)
- Part 3 Cardiovascular & Hypertension (10:00-11:00AM) ~ Tuesday November 25, 2025 ~ 2050 Weston Road (and Zoom virtually)
