Diabetes Education Program

The Diabetes Program offers patients education and counselling to help you manage your diabetes and prevent complications. 

The Diabetic team can help you with:

  • Foot assessment and care
  • Nutrition education and planning
  • How and when to take your medications
  • The importance of monitoring your blood sugar 
  • Self care education and planning that is specific to you

 Diabetes Education Sessions

Sessions are held monthly at our 245 Dixon Road and 2050 Weston Road locations

The goal is to teach you and your family about nutrition, blood sugar monitoring, medication use, foot care management and connect you with members of the Diabetes Team.
