Mental Health Addictions and Substance Use Program
We are pleased and excited to share that we will be piloting a Palliative Care Program!
This program is a new layer of service within the Mental Health/Social Work team that provides specific services to patients who have been diagnosed with or are at-risk of developing a life-limiting illness.
Any HRFHT patient who has been diagnosed with or at risk of developing, a life-limiting illness (ie. cancer, COPD,dementia, etc.) can receive this service.
The aim of this program is to provide patients and their families with ongoing social, emotional and mental health care across the illness, with a focus on heightened quality of life, wellbeing, and informed decision-making. Please call our office 416-740-2810 for more information.
A word about our group programming.
We will be moving toward delivering group classes virtually.
Patients can participate either on their mobile devices or they can phone in. In order to participate virtually, we will be asking all patients to provide us with their email address so that we can send them the link to Zoom, the platform we will be using to deliver these virtual group classes.
